BENEFIT ELIGIBILITY

All  full-time  employees  working  30  or more  hours  per  week  will  be  eligible  for benefits  on  the  1st  day  of  the  month   following  the  completion  of  the  60-day waiting period. Family  members  (Spouse  and  Depend-ent Children) are also eligible for cover-age.

Eligible Dependents include:

•  Legally married spouse

•  Child up to the age of 26 including;

−  Natural or adopted Child

−  Step Child

−  Child that you are legal  guardian

−  Child  that  has  been  placed  with you for adoption

Once  you  have  made  your  elections, you will not be able to make any chang-es until the next annual open enrollment period, unless you experience a Qualify-ing  Life  Event    (QLE)  as  defined  by  the IRS.
