Payroll Deductions
Poly Print, Inc. contributes 50% of employee and dependents medical plans and low dental plan premiums. The vision plan and life/AD&D plans are voluntary, and you will pay 100% of the premiums for these plans. Premiums are deducted from your paycheck on a before-tax basis.
Please let Human Resources know if you would prefer to pay premiums on an after-tax basis.





If you elect the High Deductible Health Plan, Choice Plus HSA, Poly Print contributes up to $60 per month toward your HSA [ Health Savings Account], to help you pay your deductible and other qualified out-of-pocket expenses.
